Step 1: Use your IUPAT Benefit Card

For most prescription drugs and dentalcare expenses, simply present your IUPAT Benefit Card at the pharmacy or dental clinic when services are rendered. You are also able to use your Benefit Card at participating Vision Care Clinics – please inquire with the clinic directly at the time of your visit.

Step 2: Submit other health claims

Vision Care, Dental Care, Drugs and Extended Health care expenses can be submitted through your eClaims account – complete your eClaims registration to access online claims submission and tracking.

Members can also submit their claims for reimbursement via regular mail. Please download and complete the appropriate form below:

• Vision Care Statement of Claim
• Dental Benefits Claim
• Supplementary Health Expense

Step 3: Register for EFT payments (optional but recommended)

To receive faster reimbursements, complete the EFT registration form and submit it to the Benefits office via fax or email.

• EFT is only available to members registered for the BPA eClaims App
• Your eClaims account will confirm once EFT has been activated

Step 4: Track your claim digitally through eClaims

Once your claim has been processed:

• You will receive notifications through your eClaims account on the status of your submitted claim.
• Once adjudicated, payments are issued the following business day via Direct Deposit and deposited into your account within 2-5 business days, depending on your financial institution.

Your claims history is available to you on the eClaims portal at any time.
