If you are a new union member, or even a current union member, you may have questions about how the IUPAT administers its benefits. We receive many questions about benefits from those just starting an apprenticeship to those who have been longtime union members. As time passes, changes are made and it’s important to always understand the impact it could make for you and/or you and your family.
First of all, we’d be remiss if we didn’t mention that those who are in unions typically have a much better benefits package and higher wages than their non-union counterparts. There are many studies out there that prove these very statements. Collective bargaining agreements help to shape your benefits package as well, and as a member of the IUPAT, we’ll ensure that you’ll get the very best deal that we can put together for you.
One way that we help administer benefits is by providing you with the necessary forms and claims information that you may need:
• If you have claims for dental, vision care, chiropractors, they must be submitted through a paper claim. However, for prescription drugs, you can apply for a Great West Life (GWL) Prescription Drug Card.
• You have a reference ID number that you must fill in for all claim forms and when making inquiries. The identification number begins with the letter B.
• Your Medical and Dental policy number is 6122.
• All correct claims should be sent to the mailing address: I.U.P.A.T. Benefit Trust Administration, P.O. Box 1280, Station B, Mississauga, Ontario L4Y 3W5.
